The Rapid Response Service is available to people leaving hospital, the service is ordered by hospital ward staff. The service provides the same as personal care and practical tasks as the mainstream service; however, it is only available for a limited period (usually up to 14 days). If you still require assistance or support after this, your service will be delivered by the Mainstream Service (or Enhanced Home Care Service if appropriate). An assessor from Social Work Services will visit you shortly after discharge to discuss your ongoing care needs.
